Form to email with link to attachments on server.

This one is on a Unix server at catalog.com and has a MySQL database avilable to it.The scripts that will work on the Unix servers, are cgi scripts, php, perl, and python. We can upgrade to CF MX 6.1 if needed

an online form with the basic contact info, name address phone fax that are required fields, several other fields for quantity info, and an optional field for "time frame needed"
A text field large enough to type a paragraph or two and leave it open so it allows as much text as possible.(don't constrain text text field to artificial limit)
The form would also allow a client to attach a data sheet or several documents instead of typing the requirments. Since we want to avoid email megabyte limits, we want the form to generate an email out with all the form info plus a link to go to the documents attached to that submitted form on the web server.
