We've experimented with two different URL Rewrite filters and are pretty happy with the Ionic URL Rewrite filter, though it doesn't support a new requirement we are looking for.
The other filter we evaluated was Helicon Lite, but it seemed kind of flaky and did not actually perform as expected. In both cases, the URL rewrite rules are defined in INI files. Unfortunately if you have a site where you want the flexibility of creating any URL you want and have it linked to a specific piece of content stored in a CMS, it becomes manual labor to have to maintain the INI file. You can always write an interface and logic to programmatically update the INI file with the URLs if necessary.
