http://surveymonkey.com/ has a great hosted survey tool.  They also have
http://www.pollmonkey.com/ which can be integrated into your page.

Both have limited free versions.

I use PhpSurveyor http://www.phpsurveyor.org/ on my personal site and
Moodle site that I use for faculty training. It was easy to install,
setup, and integrate into both sites.
