What pagination? Every browser allows scrolling. Every user knows how to scroll. Why break up a perfectly good form?
Breaking it up would certainly introduce usability problems. To refer to an earlier page means, to most people, pressing the back button. Not a good idea with a form, to my way of thinking. I say well done, Mike. One might argue with putting such a long form online in the first place debatable, but I do admire your effort. The only improvement I can imagine is the addition of some AJAX script to auto-save the document as it is filled in.
