Wow - big topic.

I'm in agreement with you on the title attribute and here are my pet peeves from sites I have visited today:

1. Poor forms markup - especially not explicitly associating labels with controls. The worst case: using an empty label in an obvious attempt to pass a validator test (e.g. http://www.med.govt.nz).

2. Verbose title attributes that don't offer anything more than the link text. Worst case "Go to link text" (e.g. http://www.odi.govt.nz).

3. Using the summary attribute on layout tables. (e.g. http:// www.studylink.govt.nz/)

4. One skip link over a deeply nested long, long, long list of links (e.g. http://www.community.net.nz/ and most other I mention).

5. Navigation links surrounding a bunch of spacer gifs, supposedly to aid screen reader users, that more or less duplicate the navigation that immediately follows it. (e.g. http://www.liac.org.nz/)

6. As above but with verbose link text. For the love of Brian, don't use the words "Go to", or "Click" in links - it's obvious, OK?

7. Ambiguous link text - with a rebel yell, she cried more, more, more - great chorus, lousy for navigating (e.g http:// www.nzqa.govt.nz/).

8. Drop down menus on websites (see many above). If you want to impress me with your card sorting skills, give me one link to your site map.

9. tables for layout - c'mon it's 2006!

10. forms require scripts to work.
