Clearly, I'm using patching in a liberal sense. If one can't build
a program without grabbing source X, you are patching X, generally
speaking. Semantics aside, my aversion to writing such an authenticator
stems from this dependency.
I'm a little surprised Bruce Guenter didn't respond as he's generally
fairly active on various mailing lists. Have you subscribed to the
vmailmgr list and posted there? Still, considering the last release
of vmailmgr was over 2 years ago, it seems unlikely anything we're
likely to want will be included and released in short order.

Yeah, if it can't find maildir, it just rewrites the last argument,
which is a pretty serious bug if you ask me.
The other alternative is to have bincimap-uidpwd take a Maildir
argument, and use it. This is in keeping with BincIMAP's claim
to be "invoked similarly". Still, there are some significant
differences. Having something like qmail-imapup would make the
claim closer to the truth.
