I have squirellmail 1.2.11 running and it creates the Send and Drafts
folders without any problems.  (I choose not to use Trash so it doesn't
try).  When initially playing with the SM config, i changed what it 
called the folders so the sent, trash and draft don't have the INBOX.
as part of the names.  Here is a snapshot from my config.php

$default_folder_prefix          = 'INBOX.';
$trash_folder                   = 'Trash';
$sent_folder                    = 'Sent';
$draft_folder                   = 'Drafts';

Because i have the default prefix set i removed it from the 
folder names as it will add it to them and not try to do INBOX.INBOX.Sent
Its been working fine for me (or atleast as far as i've been able to tell).
