I use Password Safe originally developed by Bruce Schneier and now open sourced on Sourceforge. Sync the password file among all my devices via dropbox. There are numerous versions compatible with everything from Android to iOS, Blackberry, Linux, etc. Got a couple key people here on it, but don't think it's the thing to roll out company wide.
