If you hadn’t turned off the consumer features (see http://blogs.technet.com/b/mniehaus/archive/2015/11/23/seeing-extra-apps-turn-them-off.aspx) you would see them replaced with fun things like Candy Crush, Twitter, Flipboard, and similar apps. If you set that policy to turn those off (or if your systems don’t have internet connectivity so that they can’t), these “placeholder” icons are stranded. The only way to fix that is to import your own customized start menu layout that doesn’t contain those placeholders.
