Mdt to OSD or MDT to SCCM MDT, basicly those are the same. To get the most out of it you should for sure integrate mdt to sccm. You will be able to better mimic your current mdt env. that way. As to apps, yes you would create applications or packages as needed in cm12 then simply update your tasks to use those new items or you cs.ini if doing it that way.
