I think u should go with "MPEG Relay" application. It recieves a multicast stream from a IP:Port and retransmit it on some another IP:Port.
That will work only if the incoming multicast stream is RTP. If, instead, it's raw UDP, then you can't just relay this 'as is', because few - if any - media player clients will be able to play it.
