Please look at /var/log/Xorg.0.log
If that file contains lines similar to the following:
(WW) (640x350,Generic Monitor) mode clock 31.5MHz exceeds DDC maximum 0MHz
(the "0MHz" part is relevant), then you have the same problem as in
http://archives.linuxfromscratch.org/mail-archives/livecd/2006-October/004050.html.
Please tell your monitor model, this may help me to narrow the problem down.
