The book is entirely separate from Oneiric, the form has been fixed on
the site also to say Oneiric.
Just to let you know. We lately received the Ubuntu Book. Thanks to Emily.

No, as we originally thought that book shipment would coincide with
Oneiric release but we 'll do ASAP.
Also I recall the same problem appeared with Natty CD shipment. For
both the procedures we used the forms, maybe there's a bug there?
