Reproducing the Apache license

Hi all,

It is the foundation's policy to reproduce the Apache license at the
top of each and every source file. Instead, log4j has a copyright
notice referencing the Apache license at the top of each source
file. We should start obeying the Apache-wide policy. Thus, we should
slowly start copying the license at the top of each file that does not
already have it. There is no urgency as long as we eventually do
change all files.
