As far as I can tell I would need to use a doctype in appender.xml
but this as far as I can tell is not legal?
The entity definitions (the map between entity name and a path) would
have to be in the Document (that is the top-level thing), however you
could reference the entity from anywhere. Obviously it has its
limitations and I'm not saying it is the ultimate solution, however
it is there and working.
