But will accounting and all of that work with just filter= or do I need to
call ifhp the two different ways... I notice in some printcaps it is called
as ifhp and then ofhp, and perhaps there is something that it needs to do
when called as ofhp that won't happen if I use filter=.

it's just to minimize your typework: if you use 'filter:' this means you
want this filter to be used for all dataformats, except those that you
did specify otherwise.
If you just use the 'if' entry you should be aware of files containing
different format codings in the controlfile (not 'fdfA...' but for ex
'ndfA...') as you will probably those files to be processed by your
filter too you need some additional lines in your printcap: nf=...,
rf=..., tf=..., etc.
Note that this doesn't concern the 'ldfA...' entry in the controlfile,
which NT tends to send, this means binary format and no filter can be
specified in the printcap, the file will be minimal processed, but
leading control characters will be usually ignored.
In LPRng you can force the controlfile entry to be changed by using the
'translate_incoming_format' flag, for ex 'translate_incoming_format=lf'
will replace the 'l' entry by the default UNIX format 'f' entry...
