Ahhh!!!  I see.  This is because the LPR/LPC/LPQ/LPRM programs
first decide if the user has permissions to do this, and THEN
reads the printcap file.  So the permission has to be found in
/etc/lpd.conf.

I puzzled over this a bit, until I remembered that since users are
allowed to set options like this in their 'user printcap' file
(${HOME}/.printcap) then allowing this to be done in the printcap
entry would be a security loophole.

At least by putting it in /etc/lpd.conf there is the possibility
that the Sysadmin can prevent this.
