Tom, You have the info you need. Just put "imps/2" in for the X_MOUSE_PROTOCOL in lts.conf and the ground work is complete. ZAxisMapping is hard-coded into the recent (v3) LTSP rc.setupx files, so you don't need to worry about it.
The only additional steps are telling your apps to listen for the "Z-Axis" buttons (4 and 5). This is an app-by-app thing. Some support it natively (StarOffice, Mozilla, ...) but others require some settings in each user's home directory in a config file. I'm not sure of the name off the top of my head. I think it's '.Xdefaults'...
