You are sure you have the etherboot menu cpapbility compiled in?
Usually, one should even use external menus (that's what we do) -
there is afaik something inside the mknbi-release (at least the
freshest). It's a bit more work for now, but far more extendable.
In our case, we home-brew one for we needed graphical bootmanaging.
