Are you sure that one of the two
>         hardware ethernet     00:04:75:FE:11:F5;
matches the hardware address of the client NIC?
I suggest, for testing, to include

range 10.1.1.212 10.1.1.214;
filename "/lts/vmlinuz-2.4.22-ltsp-1";

at the beginning of the group sections.
( If you may use that IP addresses and do not interfere with someone
else's dhcpd setup )

Some extract of the various log file messages you seem to receive would
have been helpful too.
