If you are already setting PRINTER_0_DEVICE = /dev/ttyS0  (or S1),
then the serial module should be loaded automatically.

perhaps you don't have the kernel modules.  They get installed as part
of the kernel install, assuming you did that using the install.sh
script.  If you just copied the kernel into /tftpboot/lts, then you
won't have the modules, in which case, you'll want to go back to
http://www.ltsp.org/ltsp-4.html and grab the latest kernel package and
install it.
