Try the RSET8139.exe program supplied on the driver floppy that came with that card. Might be you have to enable the boot prom socket before using. There's other problems that might hinder the PC from booting off the PROM, like having too large proms (generally, you need to write the same content twice into ROMs that are double the size of the ROM file, etc, or similar tricks). Checking RSET8139 would be my first guess though.
