Dave,
  Some ideas:

1) Running out of memory - I have run into flash widgets that will consume more and more and more memory.

2) Is this a Dell Poweredge with hardware RAID? There is a known issue with some Dell RAID cards that simply stop talking to the drives.

3) Are you sure the disks are getting mounted EXT3 and not falling back to EXT2?

4) Running top on the server while you run the problem flash should help you watch what is going on.
