side remark, probably not related to the cause of your problem:

normally, hyperref issues this when one adds \usepackage{hyperref} to
the source of a document and recompiles without having first cleaned
auxiliary files.

I suppose however that in your case what happened is that for some
reason the .aux file was removed, or was not produced to start
with for some reason, but the .toc file was not removed, prior
to the compilation. If there is an .aux file, then it must be
corrupted as it does not contain the 

\global\let\hyper@last\relax 

which would have avoided the above quoted warning.
