the command i gave you (i.e. java org.apache.lucene.demo.IndexHTML -create -index \jakarta-tomcat-4.0.6\webapps\index .. from some sub-directory of webapps) should work, it did on my machine.
as you can see from the above, you have to replace the {index-dir} in the command with the path to the index directory (without {} around it).
you can create a directory wherever you want (c:\lucene\index or whatever and supply the full path (in place of {index-dir}) as \lucene\index) and as long as the user that is running tomcat has read and write permission for that directory, tomcat will be able to read and write to it.

I have tried all combination.
What does {index-dir} mean? Where should I create it? How do I do to make Tomcat has permission to read and write{index-dir}?
