Had more success formating 1440 kB floppy as 1680
fdformat /dev/fd0u1680
Double-sided, 80 tracks, 21 sec/track. Total capacity 1680 kB.

Alternatively, save the image in /boot and add a line to lilo.conf like this

image=/boot/dft32_v360.img
        label=DiskTest

then run lilo, reboot and be sure to choose DiskTest option at boot up.
