Did you reformat or repartition and reformat? Try using something like gparted to delete and recreate the partition table. Otherwise it might be just faulty. Flash disks do have a limited lifespan, especially the cheap ones.

I tried formatting it from the terminal as root but no luck either it still told me its Read-only.
