Did you reformat or repartition and reformat? Try using something like gparted 
to delete and recreate the partition table. Otherwise it might be just faulty. 
Flash disks do have a limited lifespan, especially the cheap ones.

Hi Guys,
Hope your all good. A user handed to me a flash disk like a week ago
it had a virus and he asked me to help them clean it out, but I cannot
delete anything on it from my Linux box and so from a Windoze machine,
it tells me the flashdisk is read only. It doesnt have read only
switch a(i have checked more than once to confirm this and confirmed
with the owner as well). I tried formatting it from the terminal as
root but no luck either it still told me its Read-only. I have googled
and got some Windoze tools said to solve such a problem but none of
them has worked so now i throw it over you. Any ideas??  If it helps
the Flash disk is a Transcend.
