This is what I just wrote off-list to another sufferer:

I think they all work the same. Some have light, some have sound some
even a counter, but neither light or sound affect the principle at
work: a gyroscope giving the ball momentum. This makes the ball feel
heavy in your hand, as if it has a life of its own. Keeping the
gyroscope turning requires you to move your hand/wrist/fingers, and
the pull of the ball on your hand can be quite strong. It's basicaly a
wrist muscle workout, but not a static one.
Mine has sound and no counter (PB-188S).

I find a set of 'Chinese balls' to be a more gentle workout for the
fingers (very relaxing while strengthening at the same time. It
loosens up your fingers), but this leaves the wrist rather stiff.
