That's right. Since the icon themes are actually closely related to gtk, 
applications develloped with gtk are often aware of icon themes and use the 
icon from the theme (if any). However, applications are not forced to do so and 
have the freedom to choose their own icon. Most non-gtk apps like firefox and 
thunderbird are not aware of system-wide icon themes and use their own instead, 
which is why they use their default icon even if your icon theme provides a 
different one.

Also, applications can change their icon during runtime, though there are very 
few which actually do this. At least it explains why forcing theme icons on 
every applications is not necessarily a good idea.

One question: do the applications which use themed icons on their window 
titlebars also se themed icons on the taskbar? Because if that's not the case 
then my theory is wrong and you really are onto a srious bug here.
