docbook implementation II

Hi,
        from our previous discussion I have changed several things:

   * linuxdoc now supports all the different fonts as lyx.
        + typewriter
        + sans serif
        + italic
        + slanted
        + bold
        + emphasis

   * added a C preprocessor constant to the place where there are
     some arbitrary limits and commented to make it easier to change it

could you please apply this patch Jean-Marc ;-)
