Almost certainly in InsetGraphicsParams. Having said that, if you change the 
Preferences setting, it's not used to change an existing InsetGraphics at the 
moment. There's a #warning in FormPreferences.C to this effect.

What happens if you change your Preferences setting to [whatever] and then 
close, re-open your document with the "Grayscale" InsetGraphics?

If the resulting image is in Grayscale, then it's that ol' Preferences 
setting that's not being propogated. Else the bug lies in InsetGraphicsParams.

Because this was evolving code and nobody cold be bothered doing it right 
once they'd got it working-ish.
