Unfortunately this derives directly from the way it works in LaTeX.
The outer box is what provides the visible frame (except for
Frameless). The inner box scaffolding is only needed if you are not
happy with what the outer box does by default, i.e., tightly enclose
the text it contains, and you want (usually) a wider box, e.g., full
column or text width.

Or if you just want a "naked" parbox or minipage, i.e., a "Frameless",
empty-set, outer box.

I do not see an easy way to make the UI clearer to the non LaTeX
expert either. What would certainly help would be a separate menu
shortcut to "naked" minipage (and perhaps parbox).
