Yes, but only within LyX, not in the output, see the attached LyX file.
In sec. 6.6.5: "LyX will ... add commands ... that destroy the sort order." This is fixed, I think. The "italic entry at the wrong position" is in the correct position. Writing other index entries in italic works fine without @, for me. Can you to check it?
Yes, this has indeed been fixed in the meantime. I corrected it now.
In sec 6.6.6, at the end, there is a wrong index entry for xindy.
