there have been some changes in menus from version 2.0 to version 2.1:
User Guide
2.5 Document->Outline should be View->Outline Pane
3.8.3 The following 4 *View* should be *Document*
...use the menu *View* and choose View
*View*->View (Other Formats)
*View*->Update
*View*->Update (Other Formats)
6.2.1 Document>Outline should be View>Outline Pane
6.12.4 View->View Source should be View>Source Pane
A.3 The View Menu
A.3.3 View Source is now Source Pane
A.3.4 View Messges to Messages Pane
and a new entry (A.3.5 ?) : Outline Pane
A.3.5 to A.3.10, View and Update menus
they should be now in the section A.6
A.3.14 Toolbars This subsection could climb behind the panes
A.6 The Document Menu
they should have six new subsections View..., Update...
from the section A.3
A.6.3 Outline
Now it should be in A.3 The View Menu, as Outline Pane
A.9.1 The Toggle Outline button description
Document>Outline to View>Outline Pane
A.9.3 The View/Update Toolbar
All the first *View* should be *Document*
B.20 Default Output Format

Alt-V F D will View->View (Other Formats)->DVI.
to Ctrl-D will Document>View (Other Formats)->DVI.
In second and fourth paragraphs there are two *View* menu
that should be *Document*
