I had some similar problems before. You might try to rename your
.pstex_t file to something without a underscore (i.e., "_"-character), and to
edit it manually, and give the whole path to the ps file. It seems xfig
doesn't put the whole path, and lyx needs it, and also lyx doesn't like
extensions with underscores in it. This works for a fairly old version of k/lyx
and your version might have those bugs corrected, but it's worth trying. I had
exactly the same problem on lyx 0.12.
