Why table of contents?
IMO the is a visual element, you decide you where it should go, if ever,
in the same way as you decide how the title should appear, how it look...

Please show me one case where the TOC is needed when using sgml related
documents. BTW the same applies to TOF and TOT (figures and tables).

Because that is more complicated that it sounds, at least if made in a
maintainable way...
