Hello,

why you don't use fanyhdr, you can customize it!
Look at (on Linux)
/usr/share/texmf/doc/latex/fancyhdr/fancyhdr.dvi

Or have a look at:
http://www.educat.hu-berlin.de/~voss/lyx/fancy/header.phtml#fancy_1
