Help with .eps file

Linix system running Lyx version 1.1.6fix1.


I am enclosing a compressed image file (I hope that is all right!!!) that I 
cannot get to work correctly in either postscript or pdf.  It comes up 
correctly in Lyx.  It comes up in postscript with an error message. It does 
not come up at all in pdf.  

I am trying to use this in fancyhdr in pdf.  I have no clue what the problem 
is...

Is it o.k. to send out a compressed image to this list?

I appreciate any help that you can give to me.
