yes, you're right. i forget to say, that pdfpages only works
with pdflatex. so you have to export your lyx-text with
file->export->pdf (pdflatex).
if you get any problem, than export to latex (file->export->latex)
and run ba hand "pdflatex my.tex"

do you run latex pdfpages.ins from within the actual directory?
i had no problem with latex pdfpages.ins.
