I was going to send a reduced file, but noticed something interesting: 
if I removed the figures, the problem disappeared. I also found that if 
I removed the pdf bookmark command from the preamble and viewed as 
PostScript rather than PDF there was no problem (there was no difference 
between pdflatex and pstopdf, or whether bookmarks were enabled in PDF 
view).
