defining heading - text in heading

Hi everyone.
I have a quetsion about setting up a layout. Actually, I have problems with defining my heading. I am writing a two/threesided paper for lecture. I want some basic data to be printed in the heading - as follws:
the Institution:
the name of the course
the host
my name

and after that I want to have the title. I am looking for an alternative to the
predefined author, address...

I already tried out fancyhdr but got too my error msgs. Where can I find a doc
for this makro?

Is there a possibility to list all section/subsection headlines just between the
title and the text? Like a small table of contents.
