Well, it is easier to help if you tell how you want something to turn out. A 
form might look very different depending on the taste of the author. But I 
guess you need boxes and lines and such ?

There is many ways to do this, but you need to do some ERT. (Button labeled 
TeX) If you just want to frame text you can use the \fbox{framed text}. If 
you need more fancy drawing you can do this in LyX by using the \picture 
environment.

However, if the form has complicated drawings in it etc, it might be easier to 
make the form in xfig or kword and import it as a (e)ps graphics in the 
document.
