Maybe.

Actually, it's something of a conscious decision. Andre didn't want 
to force the user to use AMSMath. Maybe he uses a package that 
defines \underrightarrow that we don't know about.

LyX 1.3.x has a simple check box, 'Use AMS math'.

LyX 1.4.x has a combo box:
        Don't use AMS math
        Use AMS math
        Let LyX guess

That should keep everybody reasonably happy until someone writes a 
proper package manager. Which reminds me, someone did ;-) What 
happened to it, Herbert?
