Oh. I had tried this already, but realize now that I was trying it on a read-only copy (not checked out) of the file. What kept me from catching this (aside from not seeing the words "read only" in the title bar :-/ ) was that when I went to the Insert menu the Cross Reference item was still active (all others were greyed out).
