Do I get it right that you changed the "extension" field to "pdf4"? You have to change the "format" (to, say, "PDF4") and "GUI name" fields (to, say, "pdflatex (xpdf)") instead. The extension has to be "pdf" for all pdf files.

Yes, you got it right. So I gather that the extension field does not control the extension being used. (In other words, making it "pdf4" does not tell LyX to modify the pdflatex command to produce an output file named whatever.pdf4.)
