My table of contents has entries where the section number goes close to my 
section name. For example:

19.8. pfsync Example . . . . . . . . . . . . . . . . . . . . . . . 130
19.9. Combining CARP and pfsync For Failover . . . . . . . . . . . 130
19.10.Operational Issues . . . . . . . . . . . . . . . . . . . . . 132
19.11.Other References . . . . . . . . . . . . . . . . . . . . . . 134
 ...
A.8. pfflowd . . . . . . . . . . . . . . . . . . . . . . . . . . . 146 
A.9. pfpro . . . . . . . . . . . . . . . . . . . . . . . . . . . . 147

Also notice in the appendix entries example above how the section names 
are not lined up. Any ideas on how to fix that?

I want to add one or two more spaces after the section number (and before 
the section name), so it will print like:

19.8.   pfsync Example . . . . . . . . . . . . . . . . . . . . . . 130
19.9.   Combining CARP and pfsync For Failover . . . . . . . . . . 130
