My LyX document and the PDF are saved in the same folder, and I'm using
the following in an ERT:
    \includepdf[pages==-]{Questionnaire.pdf}

I had similar problems with includepdf. Although your Pdf and LyX files
are in the same folder, this may not be true anymore when LaTeX runs.
This happens because LyX is not aware of relative paths. Absolute ones
should be used instead.

This type of solution works. Also I have those files you
mentioned as not showing up installed with my Miktex.
Remember to have Acrobat Reader closed.
