I installed Lyx 1.4.1 on a WinXp machine, when it got to the
configuration part I got a message telling me that sh had dumped. I
found a file in the C:\Program Files\LyX141\Resources folder called
sh.exe.stackdump and it contains the following.
--------------------------------------------------------
MSYS-1.0.10 Build:2004-03-15 07:17
Exception: STATUS_ACCESS_VIOLATION at eip=71070C43

[etc.]

Of course Lyx would not run, so I put the folder that had sh.exe in
the path and then move to the resouce directory and ran sh and then
typed "./configure" it did some configuration stuff.

I have no clue about the dump, but if the configuration script ran manually then the dump may have been a one-time glitch.

I was able to run Lyx but when I open the introduction from the help I
get a dialog window with the following message

"Using the default document class, because the class book is unknown"
and of course de document does not look right. I cannot even see it
through Yap

This would seem to be two separate problems. Even if the book class is missing and LyX switched the introduction to, say, article class (which I think is the default), you should still be able to preview it in Yap. (I just did, in fact.)

Does Tools->TeX Information->LaTeX classes show a reasonably complete list of the document classes you have installed (in particular, article.cls)? If not, the configuration script probably had trouble finding your LaTeX installation, in which case it would be a good idea to run the configuration script again (from a command prompt) and capture the output generated. Near the beginning there will be messages indicating whether a working latex.exe was found.

When you say Yap would not display the introduction, was the View->DVI option present and active in the LyX View menu? If not, check Tools->Preferences...->File formats->DVI and see if the Viewer field is filled in. Perhaps the configuration script did not find yap.
