I've seen this problem when I used the TeXLive LaTeX-distribution. The
reason for this was the incompatibility between Ghostscript 7.x and
newer libraries. An update to Ghostscript 8.x fixed this (update to GPL
GS 8.14/8.15 or to GPL GS 8.54)
