Do you have the BaKoMa fonts installed? If not, you can get them (at least for Windows) from ftp://ftp.lyx.org/pub/lyx/contrib/BaKoMa4LyX-1.1.zip. Note that with Windows it's not enough to unzip the files; you need to run the Fonts applet (from Control Panel) and install them.
